Birth Certificate correct or not?

Any help would be appreciated. My fathers birth certificate shows his correct mother and father - but it shows an incorrect maiden name for the mother. The maiden name noted is the same as the woman who registered the birth. Do you think this is a mistake or could the woman who registered the birth be the mother? Hope that makes sense. Thanks for the help.
