They could have been cousins. If you have just got the certificate, it would be worth giving the GRO or the register office (wherever you bought it from) a phone call and asking if they could let you know what the maiden name is. I have done this in the past and they have let me know. If you can scan the certificate and send by email, I would be willing to have a look for you (no promises that I will be able to read it though). Let me know and I will send my email address. I have recently got my great-great grandad's birth certificate as I wanted to find out his mother's maiden name. His name was John Hurst, after his father and his mother's name is given as 'Susannah Hurst formerly Hurst' (or could be Marsh, but comparing it with what I know to be the name of the birthplace - Melling - leads me to think it's an 'H' not an 'M'). Is it reasonable to suppose that John Hurst may have married a Susannah Hurst? (Unbelievably, the registrar's name was Joseph Hurst - too many of them about!) Sadly they were cunningly married before 1837 - I estimate it to 1834/5 (making husband 19 or 20 and wife 22). I assume they were unrelated (directly, at least) as they came from different places, although in the same area.
