From my recollection: Date, time and place of death as well as cause of death....not sure what else |

1976 death certificate would have, name , age, occcupation cause of death, place of death i.e home or hospital ,informant of the death. My mothers in 1968 says I was the informant and that I was present at the death. |

I have my father's death certificate from 1976 and it also shows his date and place of birth. Also 'usual address' as well as the place where he died. |
