Normally there is a coroner's investigation and/or inquest if the person died unexpectedly.

If someone is ill in hospital or at home and being treated by the doctor and has seen the doctor regularly then the cause of death can be given by the doc.

But anyone dying suddenly, or for no apparent reason, or under suspicious circumstances would be the subject of a coroner's investigation to find out the cause of death.

The death cert should show the chap's home address which may help track down other family members.

No .It means there was a coroners inquest and he issued the document that was needed to register the death. It would have been sent to the registrar and the coroner then is recorded as the informant
