It might be on your original National Health card ?.....although I know I was issued with a new card and number some years ago.
I remember the original number, but not the new one !

Any suggestions for finding out the address I lived at the year I was born. My birth was registered where my mother was living temporarily. Which records may hold this. The year was 1947
